An exciting new concept in out of school hours care – Ambrose® Activities – is coming to St Madeleine’s in 2022.

Parents should complete this form for students who wish to participate in an accredited Elite Sports Program/Elite Arts Program/Employment in the Entertainment Industry or if there are exceptional circumstances related to taking leave.

Parents should complete this form for family holidays and travel outside of school holidays, where your child will require leave from school for 5 or more days. Once complete, please return the signed form a copy of your itinerary to the school office.

Allergic Reactions Action Plan

This action plan was designed for students with anaphylaxis. The office maintains paper copies of these forms, though, students with anaphylaxis should keep a copy of this action plan as well.
